Adolescent Resource Card
The adolescent resource card is used in all primary care visits with adolescent patients. This card is used to inform the teen about available resources for getting help with violence experienced within the home or within dating relationships. Resources were intentionally chosen that could be accessed over the Internet in order to be appealing to teens.

The adolescent resource card also includes a safety plan that is developed collaboratively with the health care professional. Safety planning includes identifying safe places, people, and ways of contacting those individuals during a time of need. Safety planning is discussed with all adolescents, regardless of whether they identify a current safety concern (or not).
